Child/Youth Volunteer Application

Thank you for your interest in serving with children here at Grace Community Fellowship!
We have a thorough volunteer screening process in order to provide a safe ministry environment.
This is our process:
1) Application (this form)

3) Criminal Background Check
4) Interview
5) Child Sexual Abuse Prevention Training
6) Ministry Policy Agreement
Only once all steps have been complete can a volunteer be cleared for work with children.

Criminal History

Child abuse is defined by the effect abusive actions have on a child, not by an adult’s intent. Child abuse describes many different types of behavior, including: physical injury that is not accidental; sexual abuse or exploitation; rejecting, abandoning or neglecting a child; exposure to drugs, alcohol or the manufacture of methamphetamine; teaching behaviors such as aggression, sexuality or substance abuse; exposure to violence; buying or selling a child.

Molestation is defined as any physical contact of a child for the purpose of sexual arousal.

Background Check Information

A criminal background check is required of all volunteers who work with children at Grace Community Fellowship.

Your Social Security Number (SSN) will be collected over the phone prior to the processing of the background check. Your SSN will NOT be stored by Grace Community Fellowship digitally or physically.

I certify that this volunteer application was completed by me and that all of the information provided in this application is true and correct to the best of my knowledge. I understand that any falsification, misrepresentation, or omission of facts called for herein will result in my disqualification from further consideration as a volunteer. I authorize any references listed in this application (but not limited to this application) to disclose any information (including opinions) that they may have regarding my history and character for this position. I waive any rights that I may have to inspect any information provided about me by any person or organization identified by me in this application. I also authorize Grace Community Fellowship to perform a background check on my person. I understand that the information provided on this application may be made available to any staff or ministry leader at this church.
